Apply online to get an income in retirement

To ensure your application is as straightforward as possible, could you please make sure you have a few documents on hand to attach to your application. Near the end of the form we’ll ask you to attach an electronic copy of the following documents:

  • Evidence of your identity (a copy of your NZ driver licence or NZ passport)
  • Evidence of your address (a copy of your bank statement, power bill, home phone bill, or rates bill)
  • Evidence of your bank details (a copy of your bank statement or bank deposit slip by a registered bank in the last 12 months)

Adviser service fees (optional)

One-off adviser service fee and ongoing adviser service fee will impact your income payments. Lifetime will reissue a quote which reflects these fees after recieving your application.

8. Your Privacy

The personal information you are providing in this application form, and in respect of any documents relating to it, including in the future, is being collected for the purposes of establishing and effectively managing your investment in the Fund and in compliance with all relevant law. The information may be used by, and disclosed to, the Manager or Supervisor, and any other entity that is involved in the administration and management of the Fund (including Inland Revenue and any regulatory body). You agree that the Manager, Supervisor and their agents may collect and use the information for these purposes, and to promote other products issued by the Manager. The information is being collected by the Manager whose addresses are in the Product Disclosure Document, and will be held by it, the Custodian and the Administration Manager (whose addresses are Level 9.34 Shortland Street Auckland, and Level 25, 125 Queen Street, Auckland) You can request access to your personal information and can ask to correct that information by calling 0800 254 338.

9. Your agreement

By completing this application form, you confirm that you have received and read the Lifetime Retirement Income Fund Product Disclosure Statement and that you agree to be bound by the Lifetime Retirement Income Fund’s terms and conditions. These are set out in the Product Disclosure Statement, online register entry, application form, and governing document.
